At our multipurpose centers, case management means walking alongside each individual to ensure their needs are met with dignity and respect. From connecting you to personalized services, we offer guidance through life's transitions and make every day a little easier.

Eligibility

To be eligible to receive case management services, you must meet two or more of the following qualifications:

65 Years or Older

Low-Income

Disabled

Living Alone
