Evidence-Based Programs

Evidence-based programs (EBPs) for older adults are structured interventions proven through research to improve health, safety, and quality of life. They commonly address chronic disease management, physical activity, mental health, and social isolation using standardized curricula delivered by trained facilitators. Fall prevention EBPs focus on reducing risk through strength and balance training, home safety education, and risk assessments, while improving mobility and confidence. Many programs include guided exercise, peer support, and behavior change strategies.

Program Information

Most programs are delivered over 6 to 12 weeks, with sessions held once or twice per week, typically lasting 1.5 to 2.5 hours. Some programs, particularly those focused on physical activity like fall prevention, may run longer to allow participants to build strength and confidence over time. These programs align with national standards supported by organizations,  ensuring they are both effective and scalable.

Vision

Our evidence-based programs for older adults include structured workshops and classes designed to support overall health and independence. Offerings may include chronic disease self-management, diabetes education, fall prevention programs such as balance and strength training or Tai Chi, caregiver support, and behavioral health programs that address depression and social isolation. Each program follows a proven, research-backed curriculum and is led by trained facilitators to ensure consistent, high-quality outcomes.
